This booking form is only for my Melbourne Guest Spot on the 15th and 16th of December.

I’ll be tattooing from Milk Bar Studio in Melbourne CBD! The address and how to get there will be emailed to you once you’ve booked an appoinment.

A deposit will be requested to secure your booking.

Custom designs will be sent to you the day before your appointment via email.

DEPOSIT I require a $50 non-refundable deposit for a standard session or a $100 non-refundable deposit for a full day session.

RESCHEDULING Make sure you give me 48 hours prior to your appointment and you can easily change your appointment date on Timely. A $50 reschedule fee will occur if you reschedule within 24 hours to your appointment.

CANCELLATION If you need to cancel your appointment, let me know via email and we can discuss your options. Your non-refundable deposit, design and appointment will be revoked if not discussed. A $50 cancellation fee will occur if you cancel within 48 hours to your appointment.
